Daxxify vs Botox: what longer-lasting results really mean

Daxxify and Botox Cosmetic are different botulinum toxin type A medicines. Daxxify has evidence of a sustained effect on frown lines, including a small direct comparison with Botox Cosmetic. That does not establish that it will last longer, look better or be the right choice for every person. The useful question is: longer by which measure, in which area, and compared with what?

This guide focuses on the lines between the eyebrows, called glabellar lines. It separates the evidence behind duration claims from the decision about your own treatment. It does not describe every use of either medicine.

Start with the exact product and treatment area

“Botox” is sometimes used for the whole category, but this comparison concerns Botox Cosmetic, containing onabotulinumtoxinA, and Daxxify, containing daxibotulinumtoxinA-lanm.

What does “lasts” actually mean?

An online timeline can describe several different things:

These distinctions matter even within one study. Someone can stop meeting a stringent response threshold while retaining some improvement. “No longer a responder” does not necessarily mean “no effect left.” Equally, some remaining improvement need not match the result that person wants.

SAKURA’s investigators measured early patient-reported change, response at a fixed visit and later duration separately. Its main assessment at week four was a prespecified study endpoint, not proof that every participant reached their personal peak on that day. SAKURA 1 and 2.

Which studies actually compared Daxxify with Botox?

The direct phase 2 comparison

The randomized 2017 study enrolled 268 adults at Canadian centers. It compared several daxibotulinumtoxinA regimens with onabotulinumtoxinA and placebo for moderate-to-severe glabellar lines. One duration measure followed an investigator-rated improvement of at least one grade until the rating returned to baseline.

The researchers reported a longer duration for one daxibotulinumtoxinA regimen than for the tested onabotulinumtoxinA regimen. However, the study was not powered to establish differences between the active-treatment groups. Its efficacy analysis excluded 77 participants who did not meet the protocol requirements, mostly because of visit timing, and the statistical comparisons were not adjusted for multiple testing.

That is useful direct evidence, but not a definitive prediction for every treatment plan or face. The tested regimens are not a conversion rule between products. Carruthers and colleagues, phase 2 study.

The pivotal SAKURA studies

SAKURA 1 and 2 enrolled 609 adults in the United States and Canada. They compared daxibotulinumtoxinA with placebo, not Botox Cosmetic. Participants received a single treatment for glabellar lines.

One combined clinician-and-patient duration measure produced medians of approximately 24 weeks. For that analysis, the event was reached when both raters’ scores returned to moderate or severe after early improvement to none or mild. The separate time until both ratings returned to pretreatment severity was longer. Neither measure means an unchanged result for six months followed by an abrupt loss of effect.

These studies establish a response under the conditions tested; they cannot establish Botox superiority or inferiority because Botox was not a comparator. Participants were predominantly women and White, which also matters when considering how broadly the findings apply. SAKURA 1 and 2 full study.

Both the phase 2 study and SAKURA program were supported by Revance, the developer. Their authors disclosed manufacturer employment and other industry relationships. Funding does not invalidate a result; it belongs alongside the methods and limitations when interpreting it. Phase 2, SAKURA.

Does the peptide formulation settle the choice?

No. Daxxify’s label describes a stabilizing peptide, RTP004, among its ingredients. Botox Cosmetic lists human albumin and sodium chloride as inactive ingredients. These are formulation facts, not proof that one produces a better expression, fewer reactions or no future loss of response. Rather than asking whether a peptide is “better,” ask what clinical evidence supports the outcome you actually want. A formulation explanation cannot answer an individual safety question on its own.

Questions to take into an assessment

Make the goal more specific than “longer-lasting.” Consider asking:

• Which line or movement are we assessing, and what would count as a useful improvement?
• What evidence applies to that area, rather than assuming frown-line findings apply everywhere?
• How will my previous result, unwanted weakness or asymmetry influence the plan?
• When will we assess the response, and how will we decide whether another treatment is appropriate?
• Who should I contact if a symptom worries me?

Bring the previous product name, treatment date and available treatment record, plus your medication and medical history. Do not change prescribed medicines yourself.
